Form I-693 Immigration Medical Exam
If you are applying for permanent residency (a Green Card) or certain other immigration benefits in the United States, you may be required to complete Form I-693, Report of Immigration Medical Examination and Vaccination Record. This exam must be performed by a USCIS-designated Civil Surgeon and helps determine whether an applicant has any health-related conditions that could affect immigration eligibility.
What Is Included?
Your appointment includes a medical history review, physical exam, vaccination review, tuberculosis (TB) screening, and blood testing for syphilis and gonorrhea as required by CDC/USCIS.
