Q: Does St Mary Adult Medicine perform medical exams for immigrants from other states?
Yes. Our US Immigration Service Certified Civil Surgeon physician is Malak G. Hermina, MD FACP. Dr. Hermina can perform exams for all states from our office at 5525 Georgetown Rd #F. Indianapolis, IN 46254.

Q: What to bring during the immigration medical exam?

You need to arrive at our facility with a Valid Photo ID (government-issued such as passport, state ID, driving license, work permit etc), Covid-19 vaccine card (if any), and your immunization\vaccination records from a medical professional or a government authorized source.

Q: Will my insurance cover the cost of the immigration medical exam?
Most major health insurance plans cover the cost of laboratory tests and vaccinations. Since we do not do any billing; we ask you to pay in full at the time of your exam. We accept cash and all major credit cards. However, we will provide all necessary documentation so you can bill your own insurance directly for reimbursement.

Q: How much does the immigration medical exam cost?

The immigration medical exam starts at $200. A complimentary (FREE) consultation with the civil surgeon regarding the additional fees for blood work or vaccines is included. The cost of the immigration medical exam including lab work starts at $395 for an Adult or $300 for a Child. This includes the physical exam, all paperwork, forms, and mandatory lab work (Chlamydia, Gonorrhea, Syphilis, and Tuberculosis via Tb-Quantiferon Gold test) with a copy of the test results, and a follow-up consultation when you pick up the results and certified forms. Additional fees may be required for additional lab work to fulfill CDC & USCIS requirements.

Q: Is Covid-19 vaccine required? What if I didn't receive the vaccine?

USCIS requires that all applicants must be fully vaccinated (2 doses of either Pzifer or Moderna; or a single dose of Johnson & Johnson). If you aren't fully vaccinated; we can still schedule your medical examination and we will guide you to a pharmacy to complete your Covid-19 vaccination for no charge.

Q: How long will it take before the results and documents are ready?
Your forms will be ready in 2 to 4 business days depending on the results from laboratory testing. Laboratory test results usually return within 2 to 3 business days (Monday thru Friday). If the Tb-Quantiferon Gold test is abnormal,  then a chest x-ray is required, which can add an additional day.

Q: Are immunizations/vaccines included in the price for my immigration medical exam?
No. The cost of immunizations/vaccines is an additional charge and can be obtained from St Mary Adult Medicine and completed at the time of your exam. They can also be taken at an outside facility. Most major health insurance plans cover vaccinations but they are an extra fee at our facility because we don't do billing. Depending on which immunizations you are required to fulfill CDC & USCIS's requirements based on age and medical condition will determine which ones must be completed before your immigration exam can be completed. Waivers can be given for certain medical conditions such as pregnancy etc

Q: Does our physician have to complete both the Vaccination Table and the Results section to properly complete the vaccination portion of the Form I-693?
Yes. US Immigration Services requires our physician to completely fill out the vaccination table and the Results section. Our physician must note in the vaccination table the complete vaccination history, date(s) of vaccinations given by us, other authorized providers (including government-issued vaccination records), and any waiver requests.
